Simply complete a short online form detailing your request—whether it’s a medical issue, prescription, fit note, or referral. Your submission will be reviewed by a GP or senior clinician and directed to the most appropriate team member, usually on the same day.
📞 No internet access? No problem. You can still call us, and a receptionist will complete the form on your behalf.
We appreciate your patience as our team adapts to this new system. We’re confident it will lead to quicker, more efficient service for everyone.
